Abstract:This paper summarizes the "study On small hepatocellular carcinoma and its extensionII in Liver CancerInstitute, Zhongshan Hospital Of Shanghai MedicaI University during the past 25 years. The results 1ndl-cated that it was an impOrtant approach to obtain long-term HCC survivOrs, of the 239 patients with 5-yearsurvival, small HCC resection accounted for 51. 4 %. It was an effective apprOach to lmprove the prognosisof HCC in the entire series, the 5-year survlval of lnpatients treated ln authorsI institution was 4. 8% in19581970, l2. 2 % in 197l 1983, and 46. 7 % in l984 1995; which were correlated to the increaseProportion of small HCC resection in the series; it was more effective as compared to large HCC resection, the 5 year survival was 6l. 3 % (n= 645 ) versus 33. 6 % (n= 950). ExtensiOns of small HCC study includ-ed early detection and treatment of small recurrent HCC, Of the l47 patients wlth re-resection, the 5-yearsurvival was 48. 9% caIculated frOm the time Of first resectiOn. Another extenslon was conversiOn Of largeHCC intO small HCC, using multimodality combination treatment, 72 out of the 663 patients wlth surgical-ly verified unresectable HCCs have been converted to resectable, 5-year survival being 62’ l %, wh1ch wascomparable tO that of small HCC resection. Studies on related basic aspect of small HCC such as cell originof recurrence, and mOlecular aspect of small HCC, indicated that biOlOgical characterlstics, particularly thetumor invasiveness, remalned the key link for further prolong survival after small HCC resection. Recent-ly, a’patient-like’ human HCC metastatic medel in nude mice has been established. Experimental inter-ventions have also been tried. Clinical trials fOr preventiOn of recurrence after small HCC resection haveshown preliminary encouraging results. However, the IIcOst-effectivenessn Of screening, the invasiveness ofHCC, the multicentric origin, the coexisted Child C cirrhosis, etc., remained great chal1enge.

Abstract:The synthesis Of DNA, RNA and protein in L121O cells was inhibited by HHO7A in a dose-dependentand time-dependent way. HHO7A shOwed much stronger inhibitory effect on [3H]-thymidine incorporationthan that on [3H]-uridine or [3H]-tyrosine incorporation. The methods of shift of absorption spectrum, shift of fluorescence emission spectrum and excitation spectrum, and isotope technique were used to investigate the mechanism of actiOn of HHO7A on DNA synthesis. The results suggested thaT the action ofHHO7A on DNA synthesis was nOt probably due to direct damage to the DNA template, it might mainlyact by interfering with the metabolism of DNA.

Abstract:To obtain high efficiency of cleavage of thrombin in fusion protein containing a ANP fusetl to Re f pep-tides,the linker sequence deslgned as VIAGR which was dlfferent from GVRGPR formerly used was stud-ied. Plasmld pHL carrying the fuslon gene Ref-NT-ANP downstream from PL Proruc)ter was derived fromexpression vector pLY1 by inserting the fragment of NT-ANP lnto lt. The exPresslon of fuslon gene waslnduced at 420,and the interested proteln Ref-NT-ANP accumlllated as inclusion bodies was lsolated bygradient centrifuge and then dissolved in 7 mol/L guanidinehydrochloride(Gdn-HCl). After dilution,renat-uration and dialyzation, the cleavage of thrombin was examined using samples with 1. 1 mol/1, (;dn-HC1and samples free of Gdn-HCl resI)ective1y. I)igestion result showed that the novel-ad()Pted cleavage sequencewas highly sensitive to thrombin when the substrate dissOlved in 1. 1 mol/I, tidn HCl. The time needed for87%cleavage (the ratio of substrate to thrombin was 5O pg/u)was less than 24 hOurs. This sequence described here which was sPecifically recognized by thrombin might be broadly applied in other fusion sys-tems.

Abstract:Exon 7 of the l’henylalan1ne hydroxylase (PAH) gene was analyzed in 15 chlldren affected wlth classicphenylketonL1rla (PKU) from northern Chlna by uslng PCRxsingle strand conformation polymorphism(PCR-SSCP) technique and DNA direct sequencing. Six missense mutatlons (l. e. R2413Q. R 241H, G247V,1,2 19H, F2541;lnd G257V )and one silent rnutatlon (V245v ) were identified. The latter three missense mu-tations were demonstrated as novel mltations in comparison with the PAH mutation database. one missense mt1tation (R241 H) was flrst dowumeTlted in Chinese. our results showed populatlon ancl reglon tllffer-ences in the PAH mutation clistribution. and suggest that there is more thfln one founding population forPKU in China. The fincling of novel mutations will enhence the molecular diagnosis of PKU.

Abstract:Monolayer culture of spontaneously contracting rat heart cells was obtained from newborn SD rats andseeded intO culture plates. 1OOTCID50 of coxsackie virus B3 (CVB3) (Nancy strain )was added as the infectedgroup. The dynamics of taurine transport across membrane of normal heart cells and changes after infectionwere examined by using radioactive isotope tracing techniques. The results demonstrated that: (1)two uptake systems of taurine with different affinities and capacities were present in the Plasma membrane of cul-tured rat cardiOmyocytes, the K. of high and low affinity were 9. 5 X 10-5mol/l, and l. 3 X 1 O 2 mol/l,,respectively; (2)taurine transport across membrane was sodium-dependent with a positive linear relationshipbetween amount of taurine influx and extracellular concentration of sOdium;in addition,it was alsO regulat-ed by osmOlarity; (3) fyalanine,a carboxylic analog of taurine, could inhibit the uptake of taurine in dosedependently; (4)taurine influx was decreased in cultured rat cardiomyocytes after infected with CVB3. (5)the lnhibition ratio of taurine transport was enhanced as the infected time prolonged. This experiment pro-vides a basis fOr further investigation of kinetic test of taurine transport and more study should be carriedout to learn the effect of taurine in viral myocarditis.

Abstract:The aim of this study is to elucidate the long term effect Of captopril treatment on the endothelial func-tion to release nitric Oxide (NO) in spontaneOusly hypertensive rats (SHR). The properties of endotheliumwere determined with a model of hindquarter perfusion in response to al adrenergic agonist, phenylephrine,at the age of 40 weeks of SHR which was administrated with captopril (100 mg/kg/day) from lntrauterineand withdrawn at 16 weeks of age. Furthermore, in the presence of N-nitro-L-arginine methyl ester(LNAME), or L-arginine, the responses to phenylephrine were studied. From the curve of perfusion pres-sure, the minimal, maximal perfusiOn pressure (PPmin,PPmax) and the maximal slOpe (slope),as well asthe 5O% of effective concentratiOn (EC,,) were obtained. The data show that in captopril treated SHR,PPmin, PPmax and slope were markedly lower, but EC50 higher than those of untreated SHR. The curveinduced by phenylephrine was significantly right shift compared with that of untreated SHR. Like WKY,the intensive reactivity to phenylephrine in the presence of LNAME was much lower than that of untreatedSHR. ln the presence of L-arginine, however, the right shift of curves was seen only in captopril treatedSHR and WKY rats, but not in control SHR. In conclusipn, endothelium does involve in the respnse ofresistant vessel to phenylephrine. The mechanism of enhanced reactivity In untreated SHR may be, at leastin part, due to the diminished capacity of producing No frpm endpthelium, and the effect pf sustained hy-potension of early captopril treatment might be relevant to the improved ability of endOthelium.

Abstract:This study was per formed to compare the efficacy of guided tissue regeneration (GTR) around dentalimplants immediately placed into extraction sockets by resorbable of nonresorbable membranes. Mandibular. P2, P3, and P4 of four aduIt beagle dogs were extracted bilaterally, and buccal standard defects were cre-ated and measured. Eighteen commercially pure titanium Steri-Oss implant fixtures were placed into thefresh extraction sockets. Four implants were untreated controls, four implants received polytetrafluoro-ethylene (e-PTFE, Gore-Tex) membranes, five implants received collagen membranes (ParaGuide), andfive implants received polyglactin 910 mesh (Vicryl). After l4 weeks, clinical measurements were takenand the dogs were sacrificed and all specimens retrieved for histologic and histomorphometric evaluation. The average gain in bone height was 2. 1mm for untreated control sites, 3. 3mm for Gore-Tex sites,3. 8mm for collagen sites, and 1. 3mm for polyglactin 910 sites. The greatest gain in bone height and volume was seen for two sites that received Gore-Tex membranes and remained covered for the entire evalua-tion interval. The results of this study indicate that Gore-Tex and collagen membrane preduced gdri re-sults for GTR around Implants immediately placed into extraction sockets. Since collagen membrane doesnot cause obvious infection and does not need the surgical reentry for membrane removal, it can be a validalternative to Gore-Tex membrane to improve bone regeneration around dental implants, while polyglactin910 mesh seems not suitable to be used as GTR membrane in immediate implantation for its hIgh infectionrate.

Abstract:Previous study has raised H’ pylori infection as a suspected biologic risk factor for gastric cancer. Acomparative case-control study involving precancerous lesions and gastric cancer was conducted inYanzhong county, an area with one of the highest rates of gastric cancer in China to study the relatlonshipbetween the H’ pylori infection and gastric cancer. Subjects in the study were all randomly selected Partici-pants of a screening program for gastric cancer sponsored by the Cancer Institute of CAMS in cooPerationwith Yangzhong county Hospital’ Totally, l25 normal controls, lO8 superficial gastritis, l l1 atrophic gas-tritis and 110 gastric cancer patients were included in our study according to endoscopy and pathology re-sult. Status of H. pylori infection was evaluated by measuring Ig G antibody in plasma with ELISA assay. Our result showed Odds ratios of H’ pylori infection were higher among gastritis and cancer groups, 4. 5(95 % CI 2. 57. 9) for superficial gastritis, 6. 3 (95% CI 3. 4 12) for atrophic gastritis, 3. 3 (95 % CI1. 95. 9) for gastric cancer. It was found in our study that consumption of pickled vegetables and drink-ing dirty water increased the relative risk of H’ pylori lnfection for both precancerous lesions and gastriccancer and that H. pylori infection had higher risk of atrophic gastritis and gastric cancer for males and alsohigher risk of atrophic gastritis for olders. Our results strongIy support the casual role played by H. pyloriinfection in the carcinogenic process of gastric mucosa.

Abstract:This study evaluated the mechanisms of 5-fluorouracil (5-FU) induced intestinal damage by investi-gating the lipid peroxide (LPO) level, myeloperoxidase (MPO) activity and disaccharidase activity in rats.Group Ⅰ animals (n=6) were sacrificed and served as a normal control group. Group Ⅱ animaIes (n= 24 )were given a single intraperitoneal injection of 5-FU (150 mg/kg) and every 8 rats were sacrificed on day1,3 and 5 after injection respectively. Results: LPO concentration in blood and intestinal mucosa was sig-nificantly higher in the group Ⅱ than in the group Ⅰ on day 1 and 3 (P<0. 05) MPO activity was signif-icantly higher in the group I than in the group Ⅰ at different times (P<0. 01 ). Lactase activity on day 5(P<0. 01); sucrase activity (P<0. 01 and P<0. 05 respectiviely) and maltase activity (P<0. 01 ) on day3 and 5, were significantly lower in the group than in the group l. Conclusion: The results indicatethat neutrophil infiltration may be involved in 5-FU-induced lipid peroxidative damage of the small intes-tine which was reflected by the decreased disaccharidases activities.

Abstract:Mechanical traction and percutaneous reduction by leverage and fixation by a bilateral groove externalfixator were performed on 23 patients with complex tibial plateau fractures involving a depressed and splitfragments. The fixator is composed of two long groove frames, three to five nuts, hooked bolts and Stein-manns pins. All fractures united in good position in three months with no incision (only pinpricks), llttle operativetrauma and no significant complications but pintract infection, and the flexion-extension range of kneemovement was excllent in all patients, so that this technique is particularly advisable for complex tibialplateau fractures.
